Samuel Smith Killed, One Hurt in Truck Accident at 27th Avenue & Lincoln Drive in Phoenix, AZ

Phoenix, AZ -- A two-vehicle collision between a pickup truck and a tractor-trailer took the life of 57-year-old in Samuel Smith and left one injured in the area of Phoenix, AZ Friday, January 20, 2017.

The incident took place at 27th Avenue and Lincoln Drive.

Eyewitnesses reportedly saw the pickup truck collide with the flatbed. The details of the incident are sparse, but officials say Mr. Smith was killed at the scene of the incident. The driver of the flatbed, who was not identified, was left with minor injuries.

The exact cause of the incident has not been determined.

The official investigation is currently underway, and authorities hope to determined more as it concludes.
