Dwight Harry Cook Killed in Work Accident in St. Augustine, FL

St. Augustine, FL -- A work incident in the area of St. Augustine, FL took the life of 57-year-old Dwight Harry Cook Tuesday, February 14, 2017.

The incident took place at Manning Building Supply in St. Augustine, FL around 12:50 p.m.

It appears the deceased victim was loading cargo of large panel doors into a truck when the incident happened. The report claims the doors for reasons unknown shifted, pinning Mr. Cook against the trailer wall. Emergency crews arrived at the scene of the incident, but were unable to save the victim.

The exact cause of the incident is unknown.

Authorities are examining the facts in the deadly incident, and hope to determined what took place at the conclusion of the investigation.
