Commentary

There aren't many details to go off of regarding this accident. There are any number of reasons which could have caused this to happen. A faulty or defective lift, improper use on the victim's part, improper use on a co-worker's part, poor safety equipment, insufficient training, etc. Without a thorough investigation, there's no way to know for sure. All I know is something went wrong so badly here that a man lost his life. Whatever caused this to occur needs to be carefully investigated and properly addressed.
