Occupational Safety and Health Administration officials indicated that 35-year-old Jose Luis Garcia was roughly 35 feet in the air on a suspended board attached to a counter-weight. Garcia was apparently working to realign aluminum tubing. While doing so, that aluminum tubing shifted in a way that somehow caused Garcia to fall. The reports say that Garcia had a harness on, but he fell to the ground and sustained fatal injuries.

Currently, the OSHA report shows two alleged rule violations determined to be "serious." There is a recommended penalty of $20,092 against the employer listed in the reports, as well. The status of these potential penalties is not clear right now. No further information about the incident can be confirmed at this time.
