From initial reports, it appears that 60-year-old Marcus Garcia was said to be assisting with a truck that was backing up. The driver of the truck then apparently lifted the dump bed to dump out material. That's when, according to OSHA, the trailer "began to tilt left." Somehow, the trailer landed on Garcia, resulting in fatal injuries.

No others were said to be injured due to the incident. OSHA officials listed one apparently "serious" alleged violation stemming from their investigations. There is also a recommended penalty of $6.250 listed in the report. No further information about the accident is available at this time.
