To be rather blunt, a truck failing to slow down is rarely the result of braking issues. Even when it is, investigations usually show the brakes were faulty due to poor maintenance, not some unavoidable defect. But is it possible something unusual led to the crash without the driver having any control over the situation? Of course. That's why independent investigators usually have the prudence to look into all possible factors, regardless of whether or not they're common. That way, they can take steps to ensure the full story is clear and that the victims have every possible opportunity to get the help they deserve.
