As unlikely as some factors may be, they still need to be investigated. Were there broken or defective crossing guards or signals? Is there a lack of signage to warn trucks about bottoming out? Is there overgrowth blocking drivers' views that the local government needs to address? Was there something unusual like a mechanical defect that led to the truck being on the tracks? As I said, it's not about what is or isn't likely. It's about making sure a victim's loved ones know they're getting the full story.
